Can I create a customized art application file upload form?

We're setting up an online gallery. We want to invite artists to respond to themes. Using your service, can we set up an online application? Do we use your art registration template and customize it? If we accept mp3 and mp4 files? or can we have them send us links to youtube or vimeo?
