Please see the attached image and respond according to the notes. I've submitted this question but I'm not getting the answer that I'm looking for. The bottom text on the form must be printed out each time, but there will be no computer entry in it. How I can I ensure that it will be on the printed version of the filled out form? Thank you for your help.

The text fields are not included in emails. However, it is possible to edit the email template in order to include your custom text.
Here is a quick example:
Another option is to get the PDFs with included text. You can get the PDFs from submissions page, and receive them as the email attachments.
1) Enable a 'Show Headers and Texts' option via form submissions page. This will include form text to the PDFs.

2) Enable a 'PDF Attachment' option in the email notification/autoresponder settings.
