How can I get the whole form sent to me after it is filled out by my clients?

I created a photography contract but when I sent a test of the form to myself, the only parts of the contract visible were the areas to be filled out.  How can I get the whole form sent to me after it is filled out by my clients?

I understand that you want to include the agreement text displayed on the form in the emails. Please be noted that the text fields on the forms are not included in the emails automatically. However, you may edit the email template of the notifications/autoresponders and add the text manually.
