Could you please provide the website URL of the form. The iframe method to embed is suggested as normally other embed method causes Javascript conflicts between the website and the form, not allowing to submit the form.
Please check once embedding the iframe code.

i am using a grace church website which is WordPress I embedded the code that the site says to use. The page is http://newresurrectionmbc.org/bowling/

I could replicate the issue, it freezes when the submit button is pressed.
It is recommended to try to choose another embed method.
Did you embed with the Wordpress 3rd party integration?

Unfortunately we can't add on plugins this website was donated to our church. The only embed codes that bring up something are the direct link and the WordPress.com but as you said it freezes. It's there another way I could try
