Add a check box in the form

Do you need it to be a basic field only when options to select? I checked your last form ID, and you seem to have another requirement with the checkbox. Please let us know, so that we can help you further. 

I have found a solution but would prefer something like:-

Statement 1      checkbox (i.e check if you agree, leave blank if you don't)

We have two methods of payment, cheque sent by mail, or bank transfer, and I want them to check the box next to their preferred payment method.
