I now have produced a registration form and now wish to send this over Facebook and furthermore to private email addresses

I would like to distribute this form to a wide audience and furthermore to put onto facebook and other sites. Please advise how I can achieve this and what I need to do to receive the submissions of the completed forms so that I can explain to the recipients.
