How do I use my Jotform account on my website?

    I created an account, gave tthe info to my website developer and they say all the forms have been created but wont show up on my website so I need to call JOTForms and request them to whitelist their IP addresses.  I cant find a number to call?  Please help.

    Please provide us with the account username or any form ID/URL from your account so that we can check it further. If you are referring to the account QualityDental, I see that it is suspended due to the fields Driver's License and SSN on your forms. It is required to display your terms on the form using the Terms & conditions widget and take the user's consent to provide such personal information through the form by adding the signature widget. Please confirm with your agreement to our terms of usage so that we may be able to activate your account.

    Yes I am referring to the QualityDental account.  How can I change the forms if I cant log into the account?  I am happy to have my website team use what ever security measures are necessary that is precisely why we chose JotForm so that we could have a HIPPA compliant means of communicating this info through our website.  I'm sorry if I am a bit ignorant personally on how this all works, can you please give me step by step instructions I am forwarding your response to my website team so that they can resolve any issues.  I was originally told by them that JotForm would need to 

    whitelist their IP addresses to fix the issues they were having, can that be done as well.

    I see that your account has been already reactivated. We have also whitelisted mentioned IP addresses.
