I tested your form and the Email Notification was successfully sent to your email address.
You can also check the email logs.
How to View All Your Form Email History
Please check your spam folder and if there are emails coming from @jotform mark them as not spam.
While I was inspecting your email logs I saw that your Autoresponder Email is not set properly. The Recipient Email of the Autoresponder Emails should be the email field. You should fix it if you want to send an email to your users when they submit the form.
