I want to run a disease registry. Basically I want to survey the same set
of rare disease patients with multiple forms. So for a group of say 50
patients, I want to send out Form 1 now, then Form 2 after 3 months, Form 3
after 6 months...
I want to be able to link all their responses by some sort of ID, maybe
name or email.
How can I do this with jotform?

You could create this workflow:
1) You could create an autoresponder on Form 1.
2) Prepopulated with basic information from Form 1 the link of Form 2, so the user avoids filling it again: https://www.jotform.com/help/351-How-to-Automatically-Pass-Form-Data-to-Another-Form
3) Put that prepopulated link of Form 2 in the Autoresponder of Form 1, and use the "Send Emails Later" function if you want to send it at a later time:
If you want to do the same for Form 3, repeat the steps:
1) Create an Autoresponder on Form 2.
2) Prepopulate the link of Form 3 with the information from Form 2.
3) Put that prepopulated link of Form 3 in the Autoresponder of Form 2, and enable the "Send Emails Later" function if you want to send it at a later time.
