How to create Test or Quiz form?

Would there be a away to have a testing option?

After each question the user would be informed if they got it right or wrong.

At the end of the test they would be shown a final score.

Each test would be require a valid email.

Email would not be able to be used more than once to take same test. To help insure user only takes test once

These are just some ideas I have come up with in my head.

However, if you want to show if the answer is correct after each question, you would need to add the Correct and Incorrect Text field to the for after each question and show them based on the selected answer with conditional logic.

You can also assign calculation values to your questions and perform a calculation to calculate the score. You can show this score on the Thank You page.

How to Perform Form Calculation Using a Widget

You can use the E-mail Validator widget to require your users to enter a valid email address. 

To prevent the users to submit the form again using the same email address, you can use the Unique Question feature.
