We have a form that is not submitting

We have only been able to make it work 1 time in the past 2 days. We need help with this. We are supposed to receive an email but it does not even show in the submissions screen.

I have just tested your form and I can see it's submitting without issues:

For example, what happens when you try to submit the form? 

Is it getting stuck or does it show an error?

Attached images via email replies will not reach this thread.
