I'd like to Create a Calibration Report

I was wondering if your software has the ability to emulate the attached document. Ideally I'd like to fill out a 'form' and have it printed out to look similar to the attachment.

I am positive that you can come up with something similar on here. You could try using the PDF Editor to create a fillable PDF Form that would look like the document on the screenshot above and would be emailed to the respondent.

Also, there is a feature that allows you to attach documents to autoresponder emails that is forwarded to respondents after they fill and submit your form.
