I am sorry you are having a problem to set up your from. Can you give us more details on what kind of form you want to set up and what problems you are running into? You are able to create a lot of different types of forms. Such as payment forms to sell products, registration, and request forms to name a few.

We have a lot of templates you can use that might be close to your requirements and then you are able to change/edit them as needed.
