Why has my account been suspended

    Good Day,


    I'd like your assistance with regards to my account. I was working on it and suddenly got logged out. I tried logging back in using my google account and even had to reset my password. Please shed some light on this. I got an error saying that my account has been suspended.


    Would appreciate your quick response to this

    Your account was suspended because the form 92436623018556 was found collecting passport information which is against our Terms of use. This is then flagged as phishing.

    Phishing. You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Hence you will need to add the below widgets in the form for us to activate your account.

    1. Terms and Conditions - https://widgets.jotform.com/widget/terms_amp%3B_condition

    2. Signature - https://www.jotform.com/help/433-How-to-Add-an-E-Signature-to-Your-Form

    The form is an application form with which a form of identification number is required. I can remove it if it violates your terms.
     
    I already added the terms and conditions widget to the above form. How do I add the E signature now given that the account suspended? Kindly advise thanks.

    Please use one of our widgets for both the Terms & Condition and E-Signature.

    I have temporarily re-activated your account so that you can add the necessary fields. Please note that you have 24 hours to do so and that we have a dedicated team and a phishing detector that checks for violations on our Terms Of Use.

    Since this is an application form, I also highly suggest that you change your email address to a corporate address to prove your legitimacy.

    you've stated.
    Am I able to update my email to the business email address during these
    24hrs?

    Yes, you can change the email address from your account settings > Profile page itself.

    Hello,
    I have updated my email address and web address as adviced, I have also put
    the terms and conditions as well as eSignature.
    Kindly confirm that it is okay so as to proceed.

    Thank you for providing us these details. Your email address is fine.

    Regarding the other details, I presume that you are referring to the Rera Fund form. Since upon checking, I could not find the Terms & Conditions widget and the Signature field added into the form. Please add these widgets into the form. Currently, you have added the Terms under a Text field and the Signature under a Short text entry field.

    Otherwise, for the form "Atlantic International Registration Form", it looks fine and I can see that you have added these mandatory widgets.
