Convert fillable PDF and use conditions

    Hello

It's been a while and I'm now trying this again.  I have a number of PDFs that make up our admission packet.

I would like to get these filled out and have some logic associated which I believe you have (conditional logic - have a field required if another field is/is not filled out, for example).

When I last tried this, I was told that the best way to get our format to retain AND have the logic applied to the form would be to make the form fillable and then upload it and use your form creator.

Currently I would be making the fillable form via PDF filler.  However, since I want to use your logic and features for HIPAA compliance then I would upload this fillable form to JotForm.

I hope this is making sense.  Could you please let me know if I have this correct and if not please correct it.

    Thanks!

Yes you are correct, you need to make your PDF into a Fillable PDF. Once you have a Fillable PDF, you can upload it to JotForm and convert it into an online form.

Once you have your online form, you can then add your conditions.
