I need to edit my form.
After it is sent, I need an the completed form to my email address AND to the #5 box, the Ambassador.
I need help editing the form! Can someone call me? 801-389-1411

Hi Barb,
If you're referring to the Launch Ambassador Request Form, I can see that the email notification is already set up. Once the form is submitted, you will be receiving an email which contains the submission data:
I may not fully understand what you mean by "#5 box", but if you mean sending an email to another person you'd want to have the submission data too, you can do that by manually adding their email address in the recipient list of the email notification. Please refer to this guide on Send-Notifications-to-Multiple-Recipients.

Box 5 is going to be a different email address each time..... we still need to have an email sent to them automatically. How can we do that?

Box 5 is the 5th 'field' that the form has. It will be a different person each time.

If you're referring to this field below, unfortunately, that is not possible since it is not an email field:
I would suggest adding an email field beneath that field and then create another email autoresponder that will use that field's value as the recipient. Here's a guide for that: Setting-up-an-Autoresponder-Email.
