The person that originally set up our Jotform account is no longer with us. We don’t know who to contact if we have any issues or questions. Are we on a rolling month to month contract? What do we pay and do we pay by credit card? Sorry to be in the blind but just really needing to get some sort of understanding.

Your account username "DriveNowAutoCredit" is registered under your email address "***e@d***enowc**d**.com". It has a Bronze yearly subscription which renews each Aug 29th.
