Autopopulate fields based based on business name

    I want to make a list of Business names and when selected the contact info automatically fills.  Right now I am using a widget but want it in a neater format 

    Carrier and receiver on my form I would like when the PDF is sent that the address is neater and cleaner format. Do I just edit the widget i am using or is there a easier way.

    You may use the Spreadsheet to Form widget. This will allow your user to input the business name, then it will read thru your spreadsheet and retrieve the corresponding row data that matches the business name. It will then auto populate all other fields you've set/

    Can you please specify which fields are you referring?

    Carrier and Receiver I want the output to look like a Standard address.

    I'm not sure it would be possible to format the output of the Grouped Values Dropdown widget on the PDF copy of the submission. But can you give us a sample of the format you are looking for to appear on the PDF attached to the email sent when your form is submitted, please?

    I need to do something like this  When Something is selected it fills in the address Fields email fields and phone.
