Problem sending booking form - multiple entries are not allowed

Salve, stiamo riscontrando problemi su un modulo per delle prenotazioni. Il problema si presenta quando si tenta di inviare più di una prenotazione (vedi screen shot allegato). Mi potete aiutare a risolvere questo problema? Grazie

Hi, we are experiencing problems on a booking form. The problem occurs when trying to send more than one reservation (see attached screen shot). Can you help me solve this problem? Thanks

To allow multiple submissions, please make sure that both 'Unique Submission' and 'Unique Question' options are set to 'No Check' in the form settings.
