why would files come to Box without proper identification?

Why would files come to Box without proper identification and instead have trim or unidentied as their names? 

Could you please share a screenshot of how the file names are displaying at your end? The cloned your form and tested the integration. It is working fine at my end. However, I am not able to test the metadata as it requires a paid Box account.

Could you please try integrating the form with Box and see if it works?
