Can I signed form be uploaded into the PDF form?

Do you want a form with a Signature field and turn the form response to PDF?

In the past, we've had a fillable form [although a crude Word document] and
because we needed signatures from a supervisor of the applicant, we would
have them get signature and upload separately. Can we upload a separate
form or give access for a separate signature? Also, are we able to
download all form info to a spreadsheet?

Signatures can be collected on online forms with Signature widgets.
