How to create payment form

Hi

I want to create a form where a person will answer questions, pay in the end and receive his order. 

I.e. Person comes on the website wants to order pizza, answers different question about their choices then in the end has to pay and receives their chosen product. So for example if the answers comes in sequence like - ABCDEFE he will receive automatically product and description ABCDEFE, then if other customer answers questions DEDDDEE he receives product and description DEDDDEE.

Would it be possible and do I need to prepare the product descriptions to all the varieties or we could use some coding to pick up from the database?
