How do I add forms/documents to my form for people to sign?

Could you please provide more information so we can assist you better? If you want people to sing, you can add a signature field to the form.

As I understand, you have a Google Document you'd like to add in your form.
You can display the document by using the Iframe Embed widget, then supply the document's public URL in the Frame URL portion.

However, please note that the document will be there for viewing purposes only. Any changes you make on the doc will not be carried over to the actual form.
If you want your users to sign, you need to add a Signature widget on your form and have them sign on your form, not on the Google Doc.
