My account has been suspended but I'm unsure as to why.

    My account has been suspended but I'm unsure as to why.

    Your account username "MIBROW8311" has been suspended due to violation of our terms of use:

    "Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    Asking for sensitive information is not allowed. If you agree to have those fields deleted, we can reactivate your account.

    Hello Michael!

    I would like to apologize for the inconvenience and I am more than happy to assist you with this.

    Like my colleague had mentioned in the earlier message, your account was suspended due to fields in your forms violating our terms of use. We do not allow users to request sensitive information like SSNs or Bank Information.

    I have gone ahead and deleted all of the offending fields from your forms Life Insurance Application and Primerica Business Application and reactivated your account. You may now access your account again without any issue.

    Your account was suspended again because the Life Insurance Form you created was still not compliant to the Terms of Use.

    You need to do the following:

    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your users sign and agree to provide such sensitive information. (I see you already added this one.)

    3. If possible, change the email address on your profile to one that shows your business domain rather than the public email address you currently show.

    We can reactivate the account but you need to agree first with our Terms.

    Your account is active again, you may update or delete that form now. If you create a life insurance form, make sure to update the email address of your account's profile with a corporate one.
