How do I change what the PDF file saves as when it is emailed to the proper recipient?
I have attached an example because it is pulling a name that is too long and not specific to the form.

To clarify it further, you would like to change your customized PDF file on the email notifications? If so, kindly check this guide: How-to-Customize-PDF-Submissions-Report.
I checked your form, and it seems that you were able to generate a PDF document. Please refer to this screencast to modify the Email attachments.
