My account was suspended for suspected phishing activity. Can I please have this reviewed and fixed?

Terms&Conditions widget — Adding-a-License-Agreement-to-a-Form

The widgets will serve as proof that your respondents agreed to have their sensitive information collected through your form.

In order for us to reactivate your account, we would need to remove the said field(s) in your form.
