Unable to access my account

    I've been trying to access my form account via my gmail account, but it only pop up that my account has been suspended. And i want to download the list of my participants that used the form registration.

    Your account was suspended due to the ID upload field in your registration form (https://www.jotform.com/form/91076600430548). We require the following widgets to be added in the form for compliance:

    1. Terms and Condition: https://www.jotform.com/widgets/terms-and-condition

    The term could be the general term from your website if you have it. Otherwise, you'll have to add the details of why you are asking such information. 

    2. eSignature field: https://www.jotform.com/help/433-How-to-Add-an-E-Signature-to-Your-Form

    We also recommend using a business email as your primary email address of the account for verification purposes. You are currently using Gmail.

    I have reactivated the account. Please make the changes as soon as you can to avoid any further suspension.
