Allow form submission more than one time

Can you please help with the error attached? 

I would like the person the submit the form every day but it seems like he can only do the submission once. 

To allow multiple submissions, please make sure that both 'Unique Submission' and 'Unique Question' options are set to 'No Check' in the form settings.
