Is there a way for us to track why our conversion rate has declined so dramatically?

    We are trying to trouble shoot why our conversation rate of completed questionnaires has dramatically declined. Is there a way to look into this on your end to troubleshoot? Is there something wrong with the questionnaire, is the submission button not working? Is there an integration issue with the form on our website?

    Hi again,

    You can use the Google Analytics or the MouseStats Form Analytics widget to understand your form respondents' behavior.

    We would like to apologize for any inconvenience. We have not received any reports of the same issue you are experiencing on your end from other users.

    Have you tried checking your form by making test submissions on your end? You can make test submissions using your form's direct link or on the website where it is embedded (if it is embedded).

    If you found out that there is an issue when submitting your form, I would suggest that you report it back to us and we will be glad to investigate it further for you.

    We have tested this multiple times. We are suspect of a few things. Does type of mobile phone cause the issue, do page load times impede in the process ??

    Mobile devices do not cause issues. JotForm is widely used on mobile equipment (for both form creation and form filling).

    do page load times impede in the process ??

    Load time does block the process. On the flip side, it does if the form doesn't load at all or is freezing.

    It has been reported by Iphone x and Iphone 6 users that the form is stalling and not advancing once it has opened to start the questionnaire from our website. Please tell me why this is happening.

    We disconnected the form as we have had too many issues with it locking up on multiple mobile phone types. Your earlier response implied it was related to load times and not at all related to mobile phone versions.
    Now we are experiencing issues with our contact form that sits on our website at:(Must be on mobile device to see this issue) You will notice the form is cut off at the top when the user reviews their input prior to submitting.) Any insight it why this is happening is greatly appreciated.

    I have tested the website you linked and the form embedded on it on an iPhone X and iPhone 6S. And here are the results of my tests.

    The Review Before Submit page of the form has its own scrollbar, separated from your website, that can be used to view all the form entries.

    Also, can you confirm if you have used the Iframe Embed Codes of the form as instructed in this guide when you embedded your form to your website?

    If you meant to send us some code, we did not receive it.

    One of the main advantages of using an iframe is that it loads as a separate webpage on the page where it's nested. Therefore, scripts inside on an iframe doesn't interfere with the scripts present on the site where it's embedded. Script conflict is often the source of bugs and glitch on the web.

When you form loads, each widget loads which may cause your form to load slower than usual. Can you check on your end if it would make a difference if you embed your form differently and if only have 1 Google Analytics widget on our form?

    I removed the widgets from every page this morning. You suggest embedding it differently, what do you recommend??

    You can embed the form to your website using different methods, such as source code, JavaScript code etc.

    I'd advise you to try each one of them and see which works the best on your site.
