New Customer seeking Info

We are a pharmacy trying to create a form or upload a current form for Vaccine Intake where it can be HIPPA Compliant and able for the patient to sign consent. The only issue is most of my patients are not Tech savy and might need something simple they can retrieve via our website as a link and finish and sign questionnaire in an easy manner.
