Hi, I am looking for a solution that can convert a PDF to a HTML web form. An individual would then be able to complete the form and all of the data would transcribe to the appropriate fields within the PDF document. Is this something we can do within your software?

Yes, you can convert your fillable PDF Form into an online form that your respondents can fill out online. Then, you can map the fields of the online form into the uploaded fillable PDF form to generate a PDF copy of the submission on the original PDF that you can download at a later time through your account.

Please note that if you have imported PDF in JotForm, you can add DocuSign widget in web form. The following guide should help you how to add widget in form: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form
If you want to add DocuSign in fillable PDF form, unfortunately it is not possible. You can only add regular signature field in PDF form. Please check the screenshot below:
