I do charity work for an organization (503c)and we have set up a form for donation in Jotform. Your platform is great. It has been easy enough to use and the form look great. I would like to be able to set the link to the form to use our domain name. It is "goacb.com". I have read articles on how to do this and configuring shouldnt be impossible to do or anything like that I am just wanting to see how do i get that ability set up for a charitable organization. What documents do you need and is there a no/low cost option for this?

I want to go from https://form.jotform.com/202805894548163 to https://www.goacb.com/202805894548163 or even https://www.goacb.com/donate

    Thanks,

    Ricky

Using custom domain and white label are only possible via Enterprise. More details on this page https://www.jotform.com/enterprise/. I'm afraid there's no other pricing except what's shown on the Enterprise pricing page. If you have any questions about Enterprise, contact them here https://www.jotform.com/enterprise/contact-sales/.

Correction: Educational institutions and nonprofit organizations are eligible for a 30% discount. Discounts are not available on HIPAA server costs. This is also mentioned on the FAQ page here https://www.jotform.com/enterprise/faq/.
