Insurance forms - asking for social security numbers

    Are you able to provide a sample terms and conditions letter or can you provide examples of a form that collects social security numbers in an acceptable manner? We are designing a form for insurance enrollments which will require SSN's, birth dates etc.. and want to make sure we are doing this securely while also adhering to all the jotform rules.

    Thank you for contacting us.

    We won't be able to provide you sample terms and condition letter, but if you want to collect social security numbers in an acceptable manner, you will have to comply with 2 things.

    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your users sign and agree to provide such sensitive information. 

    Data collected are securely stored on our server. I'd suggest looking into our security page for more information. It is also possible to apply form encryption in the form, which offers another layer of security on top of the already secured forms and data.
