If you want to provide users an offline form instead of the web form, you can create a PDF Form out of your webform. Guide: https://www.jotform.com/help/212-how-to-create-a-pdf-form
You can actually print the forms too, there's a lot of ways to do it.
1. Users can print the form with their data before submitting the form. To make it possible, add a print button on your form. Guide: https://www.jotform.com/help/24-print-button-on-forms
2. Once the user submits the form, you can print their submissions data either in the email you received, on the Tables page, Inbox page, or within the PDF document that you attached on email.
You can also find COVID form templates on our form templates gallery here https://www.jotform.com/form-templates/?classic. You can use these templates and customize them in the form builder.
