I cannot view my MH COUNSELING PROGRESS NOTE - Student/Child/Minor

    This is the note I use multiple times a day when I meet with my clients. I cannot view the note as it will not load. It is glitching everytime I try to view it since this morning at 8:45am. I can view my submissions, and I even edited one note that was from last week, but I cannot access the form for new notes. I cleared my history and cache, I tried to access the note from my mobile app, and I shut down my computer and restarted it; and I still cannot view the form to start a new note. I did edit the note today, but that was after I was already not able to access it. I was hoping that would help to access it. I tried viewing it from the sheets, and I tried viewing it from the edit screen, and still cannot access it.

    I did inform you in my original correspondence that I tried it on the app on my phone, which uses a different browser than my work computer. I tried several times throughout the day, and just again now at 11pm. It still just says loading.

    Here are the pictures from my computer, but I still cannot access it from
    my iphone either (through the all or my browser). When I go in the edit
    screen for the form there is a button in there that lets you preview the
    form, and it does the same thing; just says loading and looks like it is
    glitching and keeps trying to reload.

    Well, I would love to, but I get an UNauthorized notice that does not let
    me see the thread in the browser. This happened sometime yesterday with our
    correspondence back and forth. It says I have to switch or log in, and I
    try to log in, but it doesn't work. i am so frustrated because now I have
    to do double work for each client that I have see that needs these MH
    progress notes completed.

    Hi,

    You were receiving because the thread was set to private and it seems you were logged in to a different account in your browser. You should be able to see the thread in your browser now as I have set the thread to public and to post screenshots.

    I can return it to private if you prefer.

    Regarding your issue, I attempted to make a submission to your form and I was able to make a submission to your form.

    You should be able to see the submissions on your submissions page.

    Are you able to access the form builder in your browser and in your phone app?

    I tried it on my home internet last evening and it still did not work. I just cleared my history on Safari and tried it again and it didn’t work.

    I saw your submission. Yes I am able to access the form builder, in both browser and phone. I also tried to view the form from inside the builder, and still get the same thing. (not loading, glitching). I can access ny submissions in both browser and phone, and yesterday I even was able to edit a form. I just can't start a new form.

    so, you got me thinking. I had the form marked as private, which it has been all along, and I just changed it to public. Now it works.

    Everything is fixed! Thank you so much for your help. I am just concerned a
    little as I had everything private because of HIPAA and being a therapist
    with protected health information. I hope that no one else can access my
    files of protected health information.
