Adding multiple fields on card form

The flexibility of adding multiple fields on a single card. What they want is a checkbox that toggles a hidden textbox on the same card. I don't see any way of including customizable multiple fields other than the multi-line question widget, which doesn't accomplish what we need.

    Hi Jeff,

That's the primary function of the Card form—one question per page.

If you want to add multiple fields on a single page, use the Multi-line Question. It has a few restrictions, though; you can't use all form elements with this option. If you want to add questions, that will work.

I hope this helps. Please let us know if you need further assistance.

    Hi Sam,

The original question addressed that the multi-line question didn't address our needs. Fundamentally, we need to have a checkbox toggle the visibility of an additional field on the same card. Right now, there's nothing that gives us that capability. Is there a way to custom create a widget on our end to accomplish this?

    Hello Jeff,

You're right, that's not currently possible in the Card Form. You may consider creating a widget on your end using our Widget API here https://www.jotform.com/developers/widgets/.
