Creating an employee evaluation form that allows for manager comments

We would like to create an employee evaluation form that once submitted, would allow the manager to open it and input their comments and feedback that would not be visible to the employee, it would simply submit back to the manager. We are just trying to come up with a way to collect all the data we need from both employee and manager and have it recorded on one form. Is this possible?
