Hello - I have a form that I use for customers to populate what they would like to input when they design apparel. We then take the forms and make the apparel to their liking. The problem is this is very manual.
Is there a jotform capability/widget that could assist in developing something like the picture below digitally?
Example, I'd like for the users of the form to be able to type in attributes:
- Name
- Logo
- Numeral
- etc...

Thanks for your response. Is there a way to program fields so that I can see the responses in a spreadsheet form? Also, do you have a template available where this capability is possible?

Hi,
It is possible to view the data in a spreadsheet using Google sheets or you can download the data in excel.

Unfortunately, we don't have a template available for now. You can use the widgets recommended by @Cecile_A to create the desired form.
