Combining a form but allowing for different people to complete different portions

Hello! I am creating an Internship Contract, the first portion of the form needs to be completed by the student and the second portion needs to be filled out by the mentor that they give information on in the students' portion. They will provide an email for their mentor and I was wanting the second portion to be hidden to students but visible to the mentor so that they can complete it based on information in the first part. Is this something you can help with? As of now, they are two different forms with no way to connect them. I was wondering if I could get some help in combining them and having them communicate to the proper people.
