Let person select who that form is sent to for approval

Second question, Im trying the approval form again... is there a way for the person that is completing the form to select who that form is sent to for approval? We have a minimum of 4 people that will approve different departments requests. Id like to add their managers email address and the form would be emailed to that manager to approve once the form has been filled out by the employee.

Yes there is, please do the following:

1) Add the Dropdown field where you will list the names of the approvers, and add an Email field which should be hidden: https://www.jotform.com/help/434-how-to-hide-form-fields/

2) Create conditions to insert an email address in the Email field based on the approver that was selected: https://www.jotform.com/help/268-how-to-insert-text-or-calculation-into-a-field-using-conditional-logic/

3) Finally, create your approval flow, make sure to add the Approve & Sign type of approval if you want signatures, and assign the Email Approver field as the recipient:
