Setting up a customer submission form for pictures and video

We are looking to set up a customer submission form so that our customers can upload pictures and videos of the products.

Requirements:

• Description- customers can describe the images/video that is uploaded
• Email- requires email to submit
• Name- customer has option to submit name if they want
• Can upload high quality photos and videos
• Need a way to keep everything organized
• Would be nice to connect it to Google Drive
• If not, we need a way we can download them to post on social media
