To allow your users to fill out a form and receive a filled PDF, you have to import your PDF Form to Jotform first. Here's a guide: https://www.jotform.com/help/548-how-can-i-import-my-own-pdf-with-smart-pdf-forms/
After that, a web form will be generated out of your PDF form. You can customize your web form in the builder and make sure the fields are mapped properly to the fields in the Original PDF.
To include the Original Filled PDF in the autoresponder email for the submitters to receive after they submit the form, follow this guide: https://www.jotform.com/help/503-how-to-include-submissions-as-pdf-attachment-in-the-email-notifications/. You can also add the PDF download button in your thank you page.
