The forms are not showing the questions asked.

I have 2-issues.

1. My forms are not showing the questions asked only the response to the questions. The recipients are receiving the same.
2. I requested that all info remains on one page but the signature is on a separate page.
3. is there a better way to capture a signature, can it be typed in.

Please provide instructions on how to fix these issues.

1) Could you create a new notification email to see if the fields get updated on the new email?
