Trying to imbed form without format changes

    Hi,

     

    I am trying to have the following form appear on my website.  This form is filled out by new patients and when sent in we are then able to go to a website and download the form.

     

    I created the form by simply using the URL but it changed the format and left out all graphics.

     

    Is there a way to keep the graphics and the form exactly as it is?

    Upon checking your account I found no form there. After searching on your trash folder I found the Patient Intake Form and reinstated it.

    Anyway, I couldn't reproduce the problem you've addressed if I was able to understand it clearly.

    It might be happening because your browser isn't supporting it. Though I just tested your form in latest version of IE, FF and Chrome and the form appears just fine everywhere. Have a look at the screenshot (ignore the poor graphiacal display, i just took the screenshot in 16 colors mode to upload it quickly):

    So let us know which browser you are using. We suggest our users to use Chrome usually to avoid trouble.

    One more thing-- please clear you form's cache and browser's cache before loading your form.
