Create form that consolidate other forms

    Are you looking to add a PDF to show on your form? You can follow this guide on how to display a PDF on your form. If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?

    I HAVE CREATED SIX INDIVIDUAL JOT FORMS BUT WE WANT TO TAKE ALL SIX AND PUT THEM INTO ONE SO OUR PATIENT DOESN'T HAVE TO CLICK ON SIX DIFFERENT LINKS TO FILL OUT EACH OF THE FORMS. iF YOU KNOW OF A BETTER WAY WHERE THEY CAN CLICK ON ONE LINK AND SEE ALL THE FIVE DIFFERENT FORMS AT ONCE THAT WOULD WORK TOO.

    Hello Ann,

    Just to clarify, by PDFs, do you mean your Jotform forms? If yes, you can copy all six of your form URLs and paste them all into a Google Sheet or Word Doc located in your Google Drive/OneDrive and share the Google Sheet/Word Doc link with them. Check out this guide about Where to Find My Form URL?.

    hmm , I may not be understanding correctly but what I want to do is have one jotform so when a patient clicks on that link the jotform that they open includes the (five I previously created). So what I should have done is created one pdf that was all the forms i wanted in succession but I didn't so now that I spent so much time working on making the form look the way it did I don't want to do that work again so I am wanting to put all five jotforms into one jotform thereby having one link. I am not sure how putting all the links will do that because the customer/patient will have to open each link. is there anyway someone could call me 720-934-2442

    Thanks for further explaining your requirement. You can create a Jotform App to have multiple forms displayed on one link. Here's a guide to doing so.

    ok I will try that but I want to make sure if I have one link (with the five jotforms) that the patient/client will be able to go from one to another to another in succession , will that be the result of the one link?

    Hi Ann_Freimuth,

    You can look into Jotform App my colleague suggested to achieve this. Using Jotform Apps all of your forms will be under 1 link. User can easily select the form and fill it one after another. For this you can redirect form to another one using the redirect feature in Thank You page. You can check guide Redirecting form to another form.

    Ok i tried this but then sent myself the link and it didnt' work the way I wanted it too, is it at all possible for someone to share my screen or call me? 720-934-2442 because if you would see how this looks the forms are all listed but you have to scroll all the way down to start. is there a live chat at all . I feel like i am somewhat technologically savvy and just not getting the outcome I want

    May we ask for a screenshot of how it appears on your end so we can check further? If possible, please send us a short clip.

    Also, an alternative you can consider is to merge the forms using the Field Manager app. Here's the link: https://apps.jotform.com/app/field_manager.

    In the Field Manager app, simply select the forms that you want to merge, and select all the fields from one form and transfer to the other.

    here is how it looks at first when i click on the link i don't want it to say that or have that pop up for patients can I disable?if so how?

    this is how it looks next and see that there is no form right away I have to scroll far down to even see the start , what can I do to remedy this?

    then I have to scroll down even farther to see the rest of the forms. In addition I want the customer/patient to have to go from one form to another to another and not just stop at the first one but when I tried to test when I got to the "second one that was titled Financial policy and signed it, it just said "THANK YOU" for filling out form its been submitted and it didn't' take me to the rest. I am sure that is in the settings somewhere so please advise.

    Hi Ann_Freimuth,

    To redirect the user back to your App's homepage after the submission and select the next form to fill out, you need to update the Thank You page and redirect the user to an external URL after the form submission.

    To do it, open the form builder on each form, go to the Settings tab, click 'Thank You Page', choose the 'Redirect to an external link after submission' and paste your App's URL: https://support.jotform.com/app/221365244667156.

    ok that didnn't really work because I still have to scroll down so far to see it and I can not assume that parents (since we are a pediatric dental office) will know to do that see how much blank screen there is on the first form and also I filled out a "test" with the forms and did them and only this showed up on the link

    see how far down I have to go to scroll down on my screen, if this is what the patient sees that will not work, its after going down one entire screen height , ihave it highlighted in yellow but when i open the link i have to scroll and scroll and scroll to get to this.

    if I update the form, do i need to do a new app (with new url) to reflect the change? basically my question is if I change the form, does the url change thus having me to update it on my website

    Hello Ann,

    Thanks for getting back to us. If I understand you correctly, do you want to create a form that consolidates all the forms you created into one only? After the user submit the first page which is the first form, they will be directed to the second page which is the second form until they reach the sixth page which is the sixth form right? Can you share with us the series or title of the forms that should be first, second until the sixth form?

    Hello I created 8 forms but just want to have 5 in the order below. But can you also answer the question as to why when I 'test" out the link that the first form is so far down on the screen that I have to scroll and scroll down to even see where to start filling it out?

    Patient information form

    Financial Policy

    Consent for General Procedures

    Consent for Fluoride and Radiographs

    Acknowledgement of receipt of HIPAA

    Hi Ann,

    I have checked actually you have enabled the Show Form option in your app form settings. That is why it is open why app loads. You need to disable that Show Form option. Go to Form properties in app and turn Show form option OFF.

    After this feature OFF you will see a enclose form with title user can click it to fill out the form. Regarding 5 forms in app you can simply add the 5 forms in your form instead of 8 forms. You can drag drop forms in app to change forms order.

    ok i did what you indicated but when I "tested"it the next form did not come up in succession? it was like I still had to scroll down on the screen to find it. Are my expectations unrealistic to have the system do this? would you yourself test out the app I created and see if you see what I am seeing. It is clearly not user friendly for someone to go through this. So for example after I complete a form I am expecting it to take me to the next form I still need to complete and its not doing it, it goes back to the patient registration screen that asks me if I want to put the app on my home screen, then i have to scroll down to find the next form but it doesnt' even tell me the form I just completed. I wish someone could just call me to discuss but i know you don't do that but this is so frustrating .

    Also that "want to add app to home screen" button comes up , is there a way to turn that off?

    Thank you we are getting close but still not meeting the needs of what we want it to do

    Hi Ann_Freimuth,

    I'm sorry for the trouble this may have caused. Upon testing, it's not possible to automatically go to the next forms/pages via Jotform Apps. Your user needs to navigate your App to fill out the forms they need. You may consider enabling Mark as Completed so that your users know which form is already filled out.
