Hello volleyballsa,
I did check your form's notification email alert and did not find any issue with that. Have you already resolved your issue?
Please be noted that when you edit your email alert, it becomes dirty and any changes you make in your form will not be displayed automatically. It need to be updated manually.
The best way to solve your problem is to delete the email alert and add a new. A new email alert will always have all the form fields. The following guides should help you:
