Performing calculation then passing it to Clickup

    is there a way to create a form like this https://www.mdcalc.com/phq-9-patient-health-questionnaire-9. that scores itself and then integrates the results (total score) to click up? any creative workaround will do!


    also if it doesnt integrate to clickup but we still can get a numerical score that would be awesome.

    Also, for Clickup integration, all the mapped fields will be automatically send to Clickup.
