Hello Carmen,
I checked that form and the notification is configured correctly. I also reviewed our mail logs and found that all submissions were successfully sent to the recipient listed in the notification. The email is also not on our bounce list so you should not be having trouble receiving emails from jotform. Please check this guide on Why-Am-I-Not-Receiving-Email-Notifications for a possible reason why you're not receiving email notifications.
Also, please whitelist jotform domains and IPs in your web/email host to ensure that all the emails sent to you will be received successfully. Please take a look at the guide on Whitelisting-JotForm-IP-Addresses-and-Domains.
Check out the screenshot of the email logs:
It is possible that the emails are being routed to your SPAM folder. Please check your spam emails. If it does, please mark it as not spam so all future emails from Jotform will be sent directly into your Inbox. Also, save our emails noreply@jotform.com/ noreply@formresponse.com in your contact list. If it's not there, we suggest it best to contact your email service provider and let them check your incoming logs. If Jotform emails are being blocked, please let them whitelist them.
It is easy to enable the original PDF to be attached along with the notification email. Let me walk through the steps:
- Go to the Form settings.
- Check the option to enable original PDF attachment in the Notification email.

I also deleted the testing submission records, but now the email preferences have disappeared?

Hi Carmen,
Thank you for following our previous instructions.
Missing email preferences are usually resolved by clearing the cache on your form. Let me show you how:
- On your My Forms page, hover your mouse on your Profile Avatar icon.
- In the window that opens, click on Settings.
- In the Settings menu, scroll down to the bottom of the page and click on Clear Cache. That's it, you're done.
You can also check this guide for more information about clearing caches.

Hello Carmen,
Thanks for getting back to us. Have you also tried enabling PDF Attachment in your email notification? If not, check out the steps and screenshot below to see how it's done:
1. While you are in the Form Builder, open Settings from the top navigation.
2. Click the Emails tab on the left to configure the email settings for your form.
3. Hover on the Email Notification and click on the edit (pen) icon.4. On the configuration interface, click on the Advanced tab to navigate to advanced settings.
5. Enable the PDF Attachment option.
6. Select Original PDF with form submission data7. Scroll down to the bottom and click on the Save button to save the changes.
Give it a try and let us know how it goes.

I accidentally deleted this default table and now I'm wondering if there is
a way to get the default information back up easily?
