Questions regarding BAA account for Non-profit subscription discount

Hi, I work for a marketing agency and our client is a non profit focusing on Alzheimer’s research and discovery. Here is their site that describes the non profit study: https://saludstudy.org/about-the-study/

Could you please answer the following questions before we sign up.

Does Jotform allow for BAA doc signing?

Do forms allow for conditional logic? Meaning when a user selects a yes based on a yes/no question they are shown an additional questions with the ability to shown multiple choice answers.

Does pricing include multiple domains?

Can we send form submissions to an external domain?

Does Jotform allow for Step Forms with a progress bar? Meaning instead of having a long scrolling form we can break questions up into sections that a user can complete and click continue to the next set of questions while staying on the same form.

Would we (Alaniz Marketing) be able to sign up for Jot Form representing our non profit client (SALUD Study) at the non profit discount, or would they need to sign up directly? If so can you tell me what form of documentation you require to be approved for a non profit account?

First and foremost, we made adjustments to the subscription for the new users of HIPAA Compliance. This is currently available on a Gold subscription and Enterprise plan only, you can check the link to see the features that you benefit from and the pricing for each.

Yes, that is right. Simply enable HIPAA Compliance for your account, and you will be provided with a signed BAA as part of the process. Signing a BAA is only available when you enable HIPAA compliance on your account.  You may visit this guide for more information.

Yes, we have this, check out the sample on the screencast below:

You will need to add the Progress Bar widget to your form and you can add a page break.

For the last question about signing up for a non-profit discount. It is better if you will sign up for an account that can be accessed anytime by the person in charge of your client. You can fill out this form and this will also ask what requirement you need to provide.

Does pricing include multiple accounts? Can we have multiple domains/subdomains all under that price? Or do we need to purchase different accounts for each domain?

Thanks for getting back to Jotform. We offer two types of subscriptions, the Standard and the Enterprise. The standard subscriptions Bronze, Silver, and Gold are for a single user account only The Enterprise plan allows you to add sub-users to your account that will have the same benefit that the main account has.

The option to add a custom domain name is available just in our Enterprise plan, you can follow the steps here. May I ask what you mean by adding your own domain? Do you want your form to be embedded on your website? If so, we can help you do that. You can check this guide on how to embed your form on your site. If this is not what you mean, please let us know and provide us with some more details so we can help you achieve your requirement.
