Form as part of Form Builder

    Can you please tell me if it is possible to create the form described below and have it accessible through a Wix.com website URL?

    1. Form is in Card Form format
    2. Last element is a pdf document in which the buyer completing the form must sign, type their printed name and date. Another person (sales agent) must select their name from a single select field.
    3. When sales agent has selected their name from the single select list, the form submit button is then clicked and a copy of the signed pdf and form information is emailed to the buyer and the sales agent.

    NOTE:

    For the sales agent, we do have the option of sending the email to a central email address. A different email address would not be needed for each of the three sales agents.

    Have tried adding the iFrame Embed widget and using the embed URL for a Sign Builder document created, but the form doesn't show on the URL above like it does in preview.     Hi Cindy,

    Thanks for reaching out to Jotform Support. I've tried embedding the Signable Document on the form using the iFrame Embed widget and I was not able to replicate the issue. It works using the following URL:

    Since another person will have to continue the form, I suggest hiding the Agent selection field so it won't be seen by the form user, adding the Get Form Page URL widget, and then adding a Show/Hide Field condition to show the agent selection field once the form is edited. Here's a sample conditional rule:

    With this workaround, the form user will have to submit the form first. An email with an edit link will be sent to the Sales Agent so that he/she can edit the form to fill in the agent selection field.
